Kelly Thomas, PharmD

Kelly Thomas is an ambulatory care pharmacist practitioner specializing in heart failure management. She earned her Doctor of Pharmacy degree from the University of Wisconsin-Madison in 2023 and completed two years of residency training at SSM Health Monroe Clinic Medical Group.

At SSM Health Heart & Vascular Care located in the SSM Health Outpatient Center at 703 South Brooks Street in Madison, Kelly collaborates with patients and the cardiology team to optimize guideline-directed medical therapy and improve outcomes for individuals with heart failure. She also serves as a preceptor for pharmacy residents, fostering growth and confidence in future practitioners. An active member of the Pharmacy Society of Wisconsin, Kelly is committed to advancing pharmacy practice statewide.

Outside of work, she enjoys reading, traveling, caring for houseplants, and spending time with friends and family.
