Jen Slaughter, PharmD

Jen Slaughter graduated from the University of Wisconsin, Madison School of Pharmacy in 2011 and has over a decade of experience in ambulatory care pharmacy. She currently serves as the supervisor for the Ambulatory Care Pharmacist Practitioner team with SSM Health Dean Medical Group. In her practice, she supports both internal medicine and family medicine departments at the west Madison clinic at 752 N. High Point Road and the Waunakee clinic at 1300 S. Century Avenue. In her role, Jen helps patients improve their overall health by managing chronic conditions through collaborative practice agreements, allowing her to practice at the top of her license.

Her clinical focus includes reducing polypharmacy and optimizing diabetes care in the geriatric population. Jen is passionate about empowering patients to achieve their health goals and enjoys working in a multidisciplinary setting to build meaningful relationships with patients and care teams.

Outside of work, Jen loves trying new recipes and traveling. While she has previously served as a preceptor for pharmacy residents and students, she is not precepting at this time. Jen is an active member of the Pharmacy Society of Wisconsin (PSW) and the American Society of Health-System Pharmacists (ASHP). Her current language proficiency is in English.
