Candace Counts, APRN-CNP

Candace Counts, APRN-CNP, is a certified family nurse practitioner who specializes in cardiology, specifically congestive heart failure, coronary artery disease, and hypertension. She counsels and cares for patients with various cardiology conditions - both acute and chronic - by performing general cardiac assessments; prescribing appropriate treatments and medications; and collaborating with other providers in her practice to provide comprehensive care.

Candace prioritizes the needs, preferences, and concerns of her patients, while empowering them to take an active role in managing their health journeys. Using education and guidance, she helps them adopt healthy lifestyle behaviors and make informed decisions about their healthcare.
