Diane Lorenz, MSN, FNP-BC, is a board certi?ed nurse practitioner specializing in family practice. Diane received her post-MSN, FNP, certi?cate from the University of Missouri - St. Louis and has over 30 years of clinical experience.

Throughout her career, Diane has been a staff and clinical resource nurse with Barnes-Jewish Hospital emergency services as well as a nursing instructor. Prior to going to nursing school, Diane worked 15 years as a registered respiratory therapist.

Diane provides care to patients of all ages.

Jennifer Moore, MD, is a board eligible internal medicine physician. A St. Louis native, Dr. Moore graduated from medical school and completed her residency at the University of Missouri. In addition to her medical degree, Dr. Moore also has a master's degree in biomedical engineering from the University of Michigan.

Dr. Moore practices internal medicine for patients 18 years of age and older and has clinical interests in women's health and diabetes. She is a member of the American College of Physicians.

Dr. Abdul Rishi is a board-certified internal medicine physician who sees patients 18 years to senior age. He received his medical degree from King Edward Medical University, Pakistan and performed his medical residency at Mercy Hospital, St. Louis. Dr. Rishi was also a Research Fellow at the Mayo Clinic, Minnesota and has published multiple manuscripts in high-impact medical journals.

Jarrett Dawson, MD, is a board-certified family medicine physician. He graduated medical school from Saint Louis University and completed his residency through the Saint Louis University Family Medicine Residency program in Southwest Illinois. He is a member of the American Academy of Family Physicians and treats patients of all ages.

Dr. Dawson is interested in the full spectrum of family medicine-adults, kids, mental health, and women's health. He is also interested in bridging healthcare gaps and disparities, which disproportionately affect minority and poor communities. During medical school, Dr. Dawson worked at a student-run free clinic and helped to start a homeless outreach clinic. During residency, he cared for a mix of urban, suburban, and rural patients with high needs and low resources in a "safety net" clinic setting.

Dr. Dawson initially worked in commercial art and design, but the experience of co-founding a nonprofit in DR Congo and working with homeless children there inspired him to change course. In his free time, he enjoys relaxing with his family and taking walks in his neighborhood, as well as playing and watching soccer.

Dr. Krasnoff is board certified in internal medicine. He completed his residency and medical degree at John Hopkins University in Baltimore.

Dr. Krasnoff accepts patients 18 years and older by physician referral only.

Dr. Prager is board certified in internal medicine. Dr. Prager graduated Summa Cum Laude from Yale College earning his bachelor's degree. He earned his medical degree from the University of California San Diego and completed his residency at the University of Pittsburgh. For the past 15 years, Dr. Prager has served as an assistant professor of internal medicine at Washington University School of Medicine.

In addition to his primary care services, Dr. Prager takes a special interest in geriatric medicine and treating patients who have the human immunodeficiency virus (HIV).

Dr. Prager is fluent in Spanish and English.

Dr. Tony Toloczko is a primary care physician who treats adults ages 18 and up. His clinical areas of interest include management of chronic diseases, such as diabetes and hypertension, as well as management of complex medical needs. He is also available for internal medicine consultation.

Michael 'Mike' Kearbey, AGACNP, is an adult gerontology acute care nurse practitioner who provides comprehensive care - primary, preventative, and acute care - for adult patients ages 18 and older. He offers wellness and acute care visits, evaluating patients, developing care plans, and providing interventions for those with acute and chronic conditions. He also works to educate his patients and their families about their conditions, proposed treatment options, and care plans.

Mike decided to pursue a career in the medical field, because he wanted to carry out God's plan of helping others. He is passionate about providing quality care for his patients, ensuring they have the best outcomes and quality of life possible. He also has extensive experience in the assessment, diagnosis, and treatment of acutely ill patients in a hospital setting, effectively managing complex patient cases in high-pressure settings.
